PRIVACY POLICY

1.         Personal Information & Collection of Data

In the course of the Spinalive’s dealing with patients as chiropractors and physiotherapists, the Spinalive will request the patient to provide data and information about themselves and/or their family members (“Personal Data” and/or “Sensitive Personal Data”) to enable the Spinalive to enter into transaction or to deliver the necessary services in connection with Spinalive’s usual course of practice as a chiropractic and physiotherapy centre.

 

Such Personal Data may include information and detail concerning name, identity card/passport, identity card/passport number, age, contact number, residential address, email address, birth date, gender, occupation, bank account details, information of spouse/ parents/ children such as name and contact number, emergency contact details and other such other data deem necessary by Spinalive.  

 

Such Sensitive Personal Data may include information and detail consisting of information as to physical or mental health, medical history, condition of a data subject, religious beliefs or any other personal data.

 

 

2.         Use/Disclosure

Compilation and collection of Personal Data is relevant in connection with Spinalive’s usual practice as a chiropractic and physiotherapy centre in which event the personal data may be disclosed to relevant authorities and/or other parties in the usual course of the Spinalive’s practice including but not limited to the following purposes:-

​

a) the delivery of services whether present or future;

​

b) to facilitate payment for our services;

​

c) member or patient relationship management procedures;

​

d) Spinalive’s internal record keeping;

​

e) meeting any legal or regulatory requirements relating to the provision of Spinalive’s services and to make disclosure under the requirements of any applicable law, regulation, direction, court order, by-law, guideline, circular, applicable code; and

​

f) to enable Spinalive to send you information and/or reminder of booking reservation by email, telecommunication means (telephone calls or text messages).

3.         Confidentiality

Personal Data provided to Spinalive will be kept strictly confidential but the patient would hereby consent and authorize Spinalive to provide or disclose Personal Data to the following categories of parties:-

​

a) any person to whom the Company is compelled or required to do so under any applicable laws, regulation or in pertaining to a competent regulatory or government authority;

​

b) government agencies, statutory authorities, regulatory, governing and/or disciplinary body or other parties that would require the Personal Data for legitimate purpose;

 

c) Spinalive’s auditors, banks, accountants, insurers or other financial, legal or professional advisers and the regulatory, governing and/or disciplinary body of chiropractors.

 

Spinalive endeavour to maintain and keep such Personal Data securely and Spinalive implement a reasonable strict administration and security to safeguard in accordance with the applicable laws and regulations to prevent any unauthorized or unlawful use or disclosure of the Personal Data and steps are taken and in place to prevent accidental loss or destruction or damage to such Personal Data.

4.         Patient's Rights of Access and Correction

The patient has the rights to request access to and make correction of the Personal Data disclosed to Spinalive and in respect thereof, the patient may:-

​

a) check whether Spinalive hold or use such Personal Data and request access to such data;

​

b) request the correction of any Personal Data that is erroneous, inaccurate or incomplete;

​

c) request he Spinalive to specify or explain its policies and procedures in relation to data and types of Personal Data kept and managed by Spinalive.  

 

Written requests for access to Personal Data or correction of Personal Data or for information regarding policies and procedures and types of Personal Data handled by the Company can be made to ………………….. at enquiry.spinalive@gmail.com.

5.         Processing of Sensitive Personal Data

The patient hereby consent and authorize to the processing of the Sensitive Personal Data for medical purposes and/or healthcare services and for such other purposes deem fit or necessary by the healthcare professional.
